How to Fill Out the PSSLAI Loan Application Form (Step-by-Step)

Step 1 — Download the Correct Form

Go to psslai.com/downloadable-forms and download the form for your specific loan type — Kasangga (salary loan), Kalinga (associate member), Calamity, or Pension Loan. Make sure you are downloading the latest version.

Step 2 — Fill Out in BLOCK LETTERS

Complete all fields using black ink and block capital letters. Incomplete or illegible forms are the most common reason for processing delays. Do not leave any mandatory field blank — write “N/A” if a field does not apply to you.

Step 3 — Prepare Your Loan Amount

Step 4 — Sign All Required Sections

Sign and date all sections that require your signature. If a co-maker is required, both you and your co-maker must sign in the presence of a PSSLAI authorized officer.

Gather your valid ID, latest payslip, and any other required documents. Bring extra photocopies of everything — PSSLAI may retain copies of your documents.
