Requirements for Regular PSSLAI Membership

To apply as a Regular Member, prepare the following:

  • Other documents that may prove eligibility for membership (your unit/station may have specific requirements)
  • ₱1,000 minimum initial Capital Contribution (CapCon) deposit

Requirements for Associate PSSLAI Membership

To apply as an Associate Member (spouse or child of a Regular Member), prepare:

  • Completed PSSLAI Membership Application and Account Opening Form
  • Valid photo ID — present the original and submit a photocopy with three (3) specimen signatures
  • Certificate of Endorsement Form — must be signed by the Regular Member sponsor (available at psslai.com/downloadable-forms)

  • ₱1,000 minimum initial Capital Contribution (CapCon) deposit

How to Fill Out the PSSLAI Membership Application Form (Step-by-Step)

Step 1 — Download the Form

Go to psslai.com/downloadable-forms and download the Membership Application and Account Opening Form. Print it clearly on white A4 paper.

Step 2 — Fill Out Personal Information

Complete all personal information fields in black ink and block capital letters. Include your full legal name as it appears on your ID, date of birth, civil status, home address, and contact details. Do not leave mandatory fields blank.

Step 3 — Fill Out Employment Details

For Regular Members, fill in your agency (PNP/BFP/PPSC/NAPOLCOM), unit/station assignment, rank or position, years of service, and monthly salary. This information is used to determine your loan eligibility once you become a member.

Step 4 — Complete Account Opening Details

The form also covers your PSSLAI savings account setup — fill in your desired account type, initial Capital Contribution amount (minimum ₱1,000), and CASA deposit amount (minimum ₱100). More initial CapCon means a higher maximum loan amount later.

Step 5 — Sign All Required Sections

Sign all sections requiring your signature. Provide three (3) specimen signatures in the designated area — these must match the signature on your valid ID and will be used to verify future transactions.

Step 6 — Attach Required Documents

Attach photocopies of your valid ID and any other required documents. For Associate Members, attach the signed Certificate of Endorsement from your Regular Member sponsor and PSA documents proving your relationship.

Step 7 — Submit Your Application

Submit your completed form and documents in person at:
